Bicarbonate Gap = AG − (24 − HCO₃)

Clinical Notes

The bicarbonate gap = anion gap − (24 − measured HCO₃), where 24 mEq/L is the normal bicarbonate reference. In a single (pure) anion gap metabolic acidosis the bicarbonate gap remains near the normal anion gap (~12 mEq/L). A bicarbonate gap above ~12 suggests a superimposed metabolic alkalosis (or a baseline elevated bicarbonate); a gap below ~12 suggests a superimposed non-anion gap (hyperchloremic) metabolic acidosis.
